The required documents may differ slightly between the 16 the medical license authorits (list of them here).
We chose the documents required in the largest German state (North Rhine-Westphalia). You can see them in the following page. You can use Google Translate to help you with the translation.
By the way, the needed documents depend on where you studied university (NOT on nationality). For example, graduates from the European Union do not need konwledge exams.
